How to Apply to the Addiction Medicine Fellowship

 

Our fellowship has been filled for the 2025-2026 academic year. Our program participates in the NRMP for placement of Addiction Medicine Fellow positions and will submit their rank list in compliance with its deadlines.  If you are interested in applying to our addiction medicine fellowship, you may apply via the . 

Information included in the ERAS application must include: 

  • Medical school transcript and MSPE 

  • 3 reference letters from faculty members you have worked with in your current residency program (one letter must be from your program director).  

  • USMLE or COMLEX scores 

  • Medical school diploma 

  • Recent Photo 

  • CV

  • Personal Statement 

  • ECFMG (if applicable) 

  • J-1 Visa (if applicable) 

  • Training documentation form (to be filled out by current residency training director) 

     

Interviews will be conducted in the early summer through the end of October. Invitations will be sent via email.

Eligible candidates should have completed an ACGME–accredited, RCPSC-accredited, or AOA approved residency program prior to the start date of the fellowship. While fellows generally have a psychiatry, internal medicine, or family medicine background, we welcome physician career changers and applicants from other specialties.  

Fellows are required to be licensed or eligible for licensure in the state of Kentucky.
